For Patient Support Groups

1. Patient Support Groups must play an important role in providing essential care and support to both patients with heart disease and their families following a heart attack or cardiovascular surgery.
   
2. Patient Support Groups may vary in their objectives, but must have some of the following as their aims:
   
  establish and maintain contact with other people facing similar problems
     
  provide psychological support to both patients and their families.
     
  maintain contact with rehabilitation professionals
     
  provide information on convenient locations for exercise and leisure activities and proper utilization of these facilities
     
  provide an environment for supervised regular exercise sessions
     
  communicate with other Patient Support Groups in the area
     
  offer a visiting service to heart patients recently discharged from hospital
     
  arrange social events and gatherings
   
3. Patient Support Group applying for the Heart Support Fund must have a letter of support from a cardiologist or rehabilitation medical personnel from a medical institution in Singapore.
   
4. Each Patient Support Group seeking funding must have a minimum of 10 members.
