Patient Support Groups must play an
important role in providing essential care and support
to both patients with heart disease and their families
following a heart attack or cardiovascular surgery. |

Patient Support Groups may vary in
their objectives, but must have some of the following
as their aims: |

| |
 |
establish and maintain contact with
other people facing similar problems |
| |
|
|
| |
 |
provide psychological support to both
patients and their families. |
| |
|
|
| |
 |
maintain contact with rehabilitation professionals |
| |
|
|
| |
 |
provide information on convenient locations for
exercise and leisure activities and proper utilization
of these facilities |
| |
|
|
| |
 |
provide an environment for supervised regular
exercise sessions |
| |
|
|
| |
 |
communicate with other Patient Support
Groups in the area |
| |
|
|
| |
 |
offer a visiting service to heart patients recently
discharged from hospital |
| |
|
|
| |
 |
arrange social events and gatherings |
| |
|
| 3. |
Patient Support Group applying for
the Heart Support Fund must have a letter of support
from a cardiologist or rehabilitation medical personnel
from a medical institution in Singapore. |

| 4. |
Each Patient Support Group seeking
funding must have a minimum of 10 members. |
