Singapore Heart Foundation’s Heart Wellness Programme is a structured community-based cardiac rehabilitation programme for patients who have completed their inpatient and outpatient cardiac rehab in the hospitals, as well as individuals at risk of heart disease.
To date, we have 3 Heart Wellness Centres conveniently located across Singapore. Our centres offer a variety of facilities, equipment and multidisciplinary healthcare expertise to provide heart patients with support for life-long maintenance of their exercise habits and risk factor modification.

Heart patient* with one of the following:
* You must have completed a hospital-based cardiac rehab programme and been certified by your doctor to be fit to exercise in the community.
Individual with risk factors such as:
A referral from the polyclinic or your family doctor is required to certify that you are fit to exercise in the community.

Cardiac patients and at-risk individuals enjoy a highly subsidised fee. Cardiac patients who wish to join the SHF Heart Wellness Programme but have limited resources can approach their respective hospital’s Medical Social Worker for financial assistance.

You can benefit from our Heart Wellness Programme if you have risk factors like diabetes, hypertension, and obesity that may predispose you to a higher risk of developing heart disease. A referral from the polyclinic or your family doctor is required to certify that you are fit to exercise in the community.
As a member of the Heart Wellness Programme, you can enjoy highly subsidised exercise sessions, free nutrition consultations and health talks.
To join our Heart Wellness Programme, submit the completed referral form and call us to schedule your exercise tests, which would help our healthcare professionals assess your condition and customise an exercise programme to your endurance level.
