Singapore Heart Foundation’s Heart Wellness Programme is a structured community-based cardiac rehabilitation programme for patients who have completed their inpatient and outpatient cardiac rehab in the hospitals, as well as individuals at risk of heart disease.

To date, we have 3 Heart Wellness Centres conveniently located across Singapore. Our centres offer a variety of facilities, equipment and multidisciplinary healthcare expertise to provide heart patients with support for life-long maintenance of their exercise habits and risk factor modification.

Our fees are highly subsidised by public donations and kept affordable.

What you can expect

  • Prescribed Exercise

    Our physiotherapists will conduct a series of exercise tests to assess your condition and determine your endurance level before tailoring an exercise programme (e.g. duration, frequency, intensity and types of exercise) suited to you. As an individual’s condition and tolerance levels differ, a standard programme is unsafe and ineffective in improving a heart’s health and well-being.

    Exercise re-assessments are conducted anywhere from twice a year to once a year to track your progress. If there are changes to your exercise tolerance level, our physiotherapists will modify the programme to meet your ability for more intensive exercises. The programme will continue to change throughout your cardiac rehab journey.
  • Nutrition Counselling

    Our in-house nutritionist and dietitian will conduct an initial full assessment of your body condition, diet history, lifestyle habits, past and current medical conditions before discussing and implementing a strategy to improve your diet and lifestyle.

    Follow-ups are conducted on a case-by-case basis. If you require nutrition intervention or your physiotherapist believes you need one, a counselling session will be arranged for you.
  • Education

    We work with a diverse group of healthcare professionals to conduct regular and free educational talks that empower you with the knowledge to care for your health and well-being.
  • Support

    Singapore Heart Foundation’s Healthy Heart Support Group provides invaluable peer support to heart patients and their families adjusting to life after a heart attack and motivation to maintain healthier lifestyles.

Who is it for

Heart patient* with one of the following:

  • Heart attack
  • Coronary artery disease
  • Valvular heart disease
  • Heart failure
  • Undergone angioplasty, ballooning/stenting or heart surgery

* You must have completed a hospital-based cardiac rehab programme and been certified by your doctor to be fit to exercise in the community.

Individual with risk factors such as:

  • Diabetes
  • Hypertension
  • Obesity
  • High blood cholesterol
  • Other conditions that can lead to a higher risk of developing heart disease

A referral from the polyclinic or your family doctor is required to certify that you are fit to exercise in the community.

Our sessions are appointment-based only. Please call the Centre* at 6354 9348/63 (Bishan), 6335 9337 (Fortune Centre) or 6337 9318 (Bukit Gombak) for more information.

*(All centres) Monday – Friday, 8.30am – 5.30pm

Fees

Cardiac patients and at-risk individuals enjoy a highly subsidised fee. Cardiac patients who wish to join the SHF Heart Wellness Programme but have limited resources can approach their respective hospital’s Medical Social Worker for financial assistance.

Cardiac Patients

Once you have completed the inpatient and outpatient phases of rehabilitation in your hospital, ask the cardiac rehabilitation staff or your doctor to refer you to our SHF Heart Wellness Centre.

At-Risk Individuals

You can benefit from our Heart Wellness Programme if you have risk factors like diabetes, hypertension, and obesity that may predispose you to a higher risk of developing heart disease. A referral from the polyclinic or your family doctor is required to certify that you are fit to exercise in the community.


As a member of the Heart Wellness Programme, you can enjoy highly subsidised exercise sessions, free nutrition consultations and health talks.

To join our Heart Wellness Programme, submit the completed referral form and call us to schedule your exercise tests, which would help our healthcare professionals assess your condition and customise an exercise programme to your endurance level.
